Everyone has their own needs when it comes to health and the care they receive, and now is the time to make your voice heard.
As a community, we have the unique opportunity to provide input on the health needs of people in San Miguel County and to help identify gaps in meeting those health care needs.
The team at Tri-County Health Network will be out in the community asking residents to fill out health surveys. Your input will be key to identifying our health care needs and designing a plan to benefit us all. It is important to hear directly from you, as a member of the community, to better understand the experiences, concerns, and gaps in our region’s health care services. In October, TCHNetwork will share the results and begin collaborating with regional health care providers and stakeholders to draft a plan to address identified community needs, specifically targeting necessary resources and funding to move forward.
Please take time to support this important public engagement process. This is important to me because the health of our community is directly related to the access we have to health care plans that reflect our needs.
It will only take about ten minutes of your time to fill out the survey and all responses will remain anonymous. The survey is available online athttp://tchnetwork-health-needs.questionpro.com/, or find it at www.tchnetwork.org. Paper copies can be found at your local library and health clinics.
Don’t miss this opportunity to share your input about the health needs of your community!
